Athletic Training

ALL STUDENT-ATHLETES must complete the medical history and UIL forms prior to the start of your season to participate each year. UIL forms will be completed electronically in RankOne (linked below). Please keep in mind that this form is confidential and is for your benefit.

PHYSICAL FORM:
Needs to be completed by a Medical Doctor (incoming 7th, 9th and 11th graders). Students may pick up the Medical History/Physical form from the Head Athletic Trainer or their Coaches.

ATHLETIC/UIL FORMS:
Click the RANK ONE link above to complete the forms. PARENTS are responsible for completing the forms with their child. Please fill in information completely and correctly, this is for your Childs benefit, especially in case of an emergency. All forms are mandatory except the Chiropractic form. If you do not want your student to be treated by the Chiropractor then just skip the form. 

Athletic Training Room Rules

The athletic Training room is considered a medical facility and will be treated as such. The rules understanding signatures are in the physical packet. Failure to follow the Athletic Training Rules will result in removal from the Athletic Training Room.

**Medical Doctor Release Form

If you are seen by a medical doctor for any reason, a release form needs to be returned to the Athletic Trainer. If you are seen by a medical doctor, this removes the athletic trainers ability to provide treatment. The release form also keeps lines of communication open.

**Concussion Release Form

This form must be signed by the treating Physician, Parents and Athletic Trainer before the student-athlete may return to full athletic participation following a concussion.

Please report any and all injuries to the Athletic Trainer.
