All athletes (grade 7-12) must complete a PHYSICAL and UIL PARTICIPATION FORMS on an annual basis prior to the start of each season in order to participate in any athletic related event (including athletic periods).
This entire packet must be completed on a yearly basis prior to participation in any UIL sport. The Physical must be completed by a Physician, Physician Assistant or Chiropractor. - Go to www.registermyathlete.com
- Click Login
- Click Create an Account (You only need ONE account, even if you have children in more than one high school and/or junior high; Do Not create another account if you have used RMA in the past)
- Fill in personal account information (this should be the Parent’s personal information)
- You will be using the site as a Parent > Click Submit
After you have an account: > Login
- Under the Parents header (Blue), Click the button labeled “Click here to start/complete athlete registrations“. (the first time you log in you will be asked to agree to terms and conditions)
- Click Start/Complete a Registration (upper left hand corner of the page)
-Click Start a New Registration - this is where you will enter all of your Athlete’s information
- Click on the red bars to complete all requirements

UIL Steroid Use Acknowledgement
Most supplements are not controlled by the FDA, check the label for active ingredients before you take a supplement. You are responsible for what you put in your body. Signatures for understanding are in the physical packet.
Athletic Training Rules
The athletic Training room is considered a medical facility and will be treated as such. The rules understanding signatures are in the physical packet. Failure to follow the Athletic Training Rules will result in removal from the Athletic Training Room.
If you are seen by a medical doctor for any reason, a release form needs to be returned to the Athletic Trainer. If you are seen by a medical doctor, this removes the athletic trainers ability to provide treatment. The release form also keeps lines of communication open.
This form must be signed by the treating Physician, Parents and Athletic Trainer before the student-athlete may return to full athletic participation following a concussion.

Mrs. Herron is in her 6th year as Commerce ISD Athletic Trainer. Prior to joining the Tiger's Staff, Mrs. Herron resided as the Head Athletic Trainer at Austin College in Sherman, TX. She received her Master's of Science Degree in Athletic Training from AT Still University, Arizona School of Health Sciences in 2008, while working as the Athletic Trainer for ASU's Club Sports program. She graduated in 2006 from South Dakota State University with a Bachelor of Science Degree in Athletic Training. She is married to Curt Herron and a daughter Camdyn.
